The Details of a Claim Form


Where a creditor has a claim against an estate he or she is required to fill out the claim
form in the presence of a Justice of the Peace. The form must be submitted along with
any proof substantiating the debt to the Administrator General’s Office.

Fees schedule

The Administrator General’s (Fees) (Amendment) Regulation 2005 sets out in
accordance with the law a schedule of the fees charged by our department. It details the
functions and the amount charged.

Form of Particulars

This document is required by the administrator General for Jamaica to initiate
investigations into the assets owned by the deceased person. The form of Particulars is
also utilized to identify the possible beneficiaries of the deceased estate.

Affidavit in proof of death

This is a sworn document lodged with other documents to secure a Grant of Letters of
Administration for a deceased person’s estate. The affidavit is used mainly to support the
claim that the person in whose estate the grant is sought is deceased.
